About Us
Our mission is to improve healthcare for patients experiencing Chronic illnesses and prevent hospital readmissions, between doctor visits. TC Health 360 has an unorthodox approach that offers care coordination between the physician’s office and the patients home. This approach has improved the care for patients and provided additional reimbursements to Physicians, for services TC Health 360 provides.
Our vision is to help Patients become more engaged in their healthcare and to educate Physicians on how to maximize their reimbursement, by reducing hospital readmissions and increasing program participation, for patients in need of Chronic Care Management services.
Our History
TC Health 360 is a team of healthcare professionals that started working together exclusively in 2019.
After working with countless Medical Practices, we’ve determined why patients were returning to the hospitals, while battling Chronic illnesses. During the Covid-19 pandemic in 2020, we immediately started working remotely with CCM patient’s and created various strategies with positive outcomes.
Our data showed that Physicians did not have the support to keep patients engaged in managing their own healthcare needs, while keeping up with the ever-changing regulations in healthcare. After years of research, TC Health 360 created services and support to offer patients the care coordination between office visits, while increasing patient engagement and
maximizing the Physicians reimbursements, without any additional work added to the Physician. In fact, the services provided, reduced the stressors on Physicians, to allow for a more productive delivery of
healthcare services to their patients. With all the successes of the TC Health pilots, we decided it was time to provide these services to many more.
